Referrals

Empowered Voice Rehabilitation works with physicians and non-physician providers to provide evidence-based treatment in-person or via telehealth.

  • Physicians refer patients who display one or more of the following conditions:

    • Dysphonia

    • Dysphagia

    • Chronic cough

    • Gender dysphoria related to voice

    • Paradoxical vocal fold motion (PVFM)/Vocal cord dysfunction (VCD)

    • Exercise-Induced Laryngeal Obstruction (EILO)

    • Pre-radiation/chemo for head & neck cancer

    • Dysarthria

    • Aphasia

    • Traumatic Brain Injury

    • Concussion

    • Long-COVID

    • Chemobrain

    • Cognitive-communication disorder

    Physicians can refer to Empowered Voice Rehabilitation when a patient is exhibiting any voice, swallowing, breathing, resonance, language, communication, or cognitive concerns. If a patient is wanting to utilize insurance for coverage of their sessions, they will need to have a referral from a provider. If the patient is planning to do private pay, they do not need a referral from a physician, but it is helpful to have one on file. Alternatively, the patient can directly contact Empowered Voice Rehabilitation.

  • If you are wanting to utilize insurance for coverage of your sessions, you will need to have a referral from a provider. If you are planning to do private pay, you do not need a referral from a physician.

  • Physicians must complete a prescription or referral form. The prescription or referral form must specify: speech-language pathology evaluation and treatment with diagnosis and ICD-10 code(s). Physicians may use their own referral forms or they can download our referral form using the button above. Referrals can be faxed to Empowered Voice Rehabilitation at (904) 395-2255.

    Empowered Voice Rehabilitation will complete an assessment and prepare a recommended plan of care for review and approval by the prescribing physician/medical provider.

    Treatment will be completed when physician approval has been received.
