Billing & Insurance
Please bring all insurance cards with you at the time of your appointment or upload them to the Practice Better portal prior to your initial evaluation. This will help you receive optimal coverage from your insurance carrier.
As a courtesy to our patients, Empowered Voice Rehabilitation LLC will work with insurance providers with whom we have contracts. If your insurance plan is not on our list, we will be happy to assist you in determining if you qualify for out-of-network coverage for your visit. If Empowered Voice Rehabilitation LLC is out of your insurance plan network, it is typical that your insurance will pay a smaller percentage of charges leaving a larger percentage of those charges as the responsibility of the guarantor.
Empowered Voice Rehabilitation LLC will file your claim(s) to your insurance company. If you have any questions regarding insurance pre-certification, your account, or the status of the insurance claim filing, please contact us at (904) 539-9930 or by fax at (904) 395-2255.
Covered insurance amounts will be submitted to your insurance company as a courtesy. Once a determination of benefits is made by your insurance plan, remaining patient amounts become due and payable upon receipt of the statement sent to you by Empowered Voice Rehabilitation LLC. Co-pay and deductible amounts are expected at the time of service.
Our Procedures & Professional fees
Empowered Voice Rehabilitation LLC prides itself on transparency. Our professional procedures fees are posted here to assist those patients who prefer to pay out of pocket for their medical care, or who have high deductible coverage and have not yet met their deductible. We have included our most common codes for orientation and estimation purposes, and not as a guarantee of charges. Also, please note that these fees are subject to change.
Given the nature of our practice, some patients may be billed for more than one code.
