  • At Empowered Voice Rehabilitation we treat the whole person. We do more than treat the disorder because each individual is more than a diagnosis. Through education, support, and compassion we provide person-centered care that aims to improve quality of life and generate meaningful improvements.

    Clinicians at Empowered Voice Rehabilitation have extensive experience in the medical field. They have worked in various medical settings and understand the complexities of medical issues. Our clinicians are professional and knowledgeable, while providing up-to-date evidence-based and trauma-informed care.

    We know that caregivers, loved ones, friends, and other communication partners play a vital role in your care and well-being. We support these caregivers by providing education and resources to all members of your team.

    If your physician has sent us a referral, we will contact you to schedule.

  • If you are looking to have your services covered by insurance, we need to have a referral from a physician.

  • Empowered Voice Rehabilitation does not have a physical office location. At Empowered Voice Rehabilitation, we believe that functional treatment is integral to achieving goals. We offer mobile services in-person in a patient's own home (for patients located in the greater Jacksonville, FL area), at their physician's office (if requested by the physician), via tele-practice, or out in the community. When appointments are scheduled, we will discuss and confirm location with the patient prior to the appointment.

  • At Empowered Voice Rehabilitation, we believe that functional treatment is integral to achieving your goals.   • Empowered Voice Rehabilitation currently accepts:

    • Traditional Medicare

    • FL Medicaid (for individuals under the age of 21)

    • Green Mountain Care

    • First Health Network

    • Tricare

    • Private pay

    We have attempted to contract with other insurers, but many of the other insurance companies (FL BCBS, Aetna, Medicare Advantage plans, etc.) have closed panels for Speech-Language Pathologists. Due to this, we are an out-of-network provider for these insurance companies. A patient can check to see if they have out-of-network benefits through their insurance company if we are not currently contracted with them. We are happy to bill as an out-of-network provider or provide the patient with a superbill for potential coverage through other insurance carriers. We suggest that patients check with their insurance plan to see that they will accept a superbill. Payment is due at the time of service and we accept checks, cash, credit or debit cards, and HSA or FSA cards. Check our website or contact us if you would like to receive updates to our insurance coverage options.

  • Empowered Voice Rehabilitation will always complete a health insurance verification and provide you with a Good Faith Estimate prior to initiation of services so that you know what their patient responsibility of the total cost will be and can weigh this against cost for in-network providers. Because we provide mobile services, you do not need to fight traffic to get services and treatment can be provided in the most functional setting. Additionally, as a specialist we can provide higher level care than a generalist speech-language pathologist that you might find at a traditional clinic or rehabilitation center. If you decide that you'd like to find an in-network provider, we are happy to help you locate a provider.

  • Yes! You can use an FSA or HSA card to pay for these services.

  • A superbill is an itemized list of all healthcare services provided to a client. These claims can be submitted to payers for reimbursement. The superbill will also contain additional information about the client visit that is required by insurance companies for reimbursement.

    Superbills differ from receipts or invoices because they provide additional information regarding the visit (diagnosis and procedure codes) that are needed to get a claim approved and reimbursed.

  • At this time, we are only licensed to treat patients residing in the states of Florida and/or Vermont.

  • Practice Better is the practice management platform that Empowered Voice Rehabilitation utilizes. When you become a client with Empowered Voice Rehabilitation, you will receive a link to join Practice Better. This will give you access to your intake forms, medical record, notes from sessions, secure HIPAA compliant chat with your therapist, HIPAA compliant video conferencing for televideo appointments, and assigned home practice tasks.
